リトラクションや伸展が症状を軽減する力学的負荷方向（directional preference: DP）となる頚部症状を持つ者に対して、リトラクションや伸展の反復運動で症状の軽減がみられるかを検証すること

介入群は、リトラクションもしくは伸展のうち、最も症状の軽減を引き起こ
す方を疼痛自制内で反復するホームエクササイズを1-３時間おきに10-15 回行う。1-2 週ごとに回ホームエクササイズの方法の確認・修正を行う。

The intervention group will undertake home exercises of retraction or extension of the neck 10 to 15 times every 1-3 hours. The exercises will be corrected and modified once or twice a week.

埼玉県立大学の学生
マッサージや湿布などの対症療法以外の治療介入を受けていない
Neck Disability Indexが16％以上
外傷既往や脊柱もしくは上肢の手術既往がない
合併症がなく頚部症状以外は健康な者

Neck symptoms for more than a week
Students in the Saitama Prefectural University
No intervention other than massage or pain relief patch
Neck Disability Index is 16% or higher
No history of trauma or surgery on the spine or upper limbs
Those who have no complications and are healthy except for the cervical symptoms

結果：各群20人が試験を完了した。経過観察と比較したMDTの効果は、無作為化後6週間の時点で十分に大きかった。

参加者背景/Baseline Characteristics

組み入れ基準は、(1)18～60歳の大学生、(2)合併症がなく、頸部症状以外は健康な人、(3)頸部症状が1週間以上ある人、(4)頸部障害指数（NDI）が16％以上であり、日本における臨床的障害のレベルを超える状態であることを示すこと、(5)脊椎や上肢に外傷や手術歴がない人、(6)マッサージと湿布以外の介入を行わない人とした。除外基準は以下の通りである： (1)ベースライン評価時に頸椎の後退または伸展のDPがないこと、(2)介入期間中に医療機関で治療を受けていること。

The inclusion criteria were (1) university students aged 18 to 60 years, (2) those who had no complications and were healthy except for neck symptoms, (3) neck symptoms for 1 week or more, (4) neck disability index (NDI) of 16% or more, indicating a condition that is above the level of clinical disability in Japan, (5) no history of trauma or surgery on the spine or upper limbs, and (6) no interventions other than massage and compresses. The exclusion criteria were as follows: (1) absence of DP for cervical retraction or extension during the baseline evaluation and (2) receiving medical treatment at medical institutions during the intervention period.
