目的/Objectives
目的1/Narrative objectives1	日本のICUでのせん妄の発症や重症度の変化にともない、どの程度医療コスト(入院費や医療資源投入量)を増減するのかを明らかにすることを目的とする。	The objective of this study is to determine the extent to which medical costs (hospitalization costs and medical resource inputs) increase or decrease with the onset and severity of delirium in Japanese ICUs.

選択基準/Key inclusion criteria	1)2016年4月1日以降に入院し、2018年3月31日までに退院した患者　　　　　　　　　　　　　　　　　　　　　2)川崎医科大学附属病院：心臓血管外科手術後、ICUに入室した患者 3)川崎医科大学総合医療センター：食道がん手術後、ICUに入室した患者	1)Patients admitted on or after April 1, 2016 and discharged by March 31, 2018.2)Kawasaki Medical School Hospital: Patients admitted to ICU after cardiovascular surgery. 3)Kawasaki Medical School General Medical Center: Patients admitted to the ICU after esophageal cancer surgery
除外基準/Key exclusion criteria	1)20歳以下 2)精神疾患の既往がある患者	1) 20 years old or younger 2) Patients with a history of mental illness

主な結果/Results	2病院で対象者122名 年齢72.50±10.81歳 心臓血管術後患者106名(86.9％) 男性80名(65.6％) 非せん妄(NDg):26.2％ Subsyndromal delirium(SDg):43.4％ せん妄（Dg）:30.4％ 医療費 NDg:34648.30±12504.61USD SDg:45596.54±19385.21USD Dg:53546.04±19546.7USD 全群間比較において有意差あり(P<0.01)、せん妄重症度上昇により医療費が増加した。 入院期間 Dg22±11.36日(10-55)、SDg27±20.42日(10-117)、Dg31±32.00日(12-160)であった。NDgはDgとの比較において有意差(p<0.01)あり。ICDSC4点以上で入院期間が延長した。	2 hospitals122 participants Age 72.50years(Median) Cardiovascular surgery:86.9% male patients:65.6% Non delirium(NDg):26.2% Subsyndromal delirium (SDg):43.4% Delirium(Dg):30.4% Medical costs(Median) NDg:34648.30USD SDg:45596.54USD Dg:53546.04USD There was a significant difference in the comparison between all groups(p<0.01), Length of hospitalization(Median) NDg 22days,SDg 27days,Dg 31days.NDg was significantly different from Dg(p<0.01).

その他関連情報/Other related information	病院医事課に調査対象患者で示した条件に該当する患者をリストアップし、カルテNo及びDPCデータより患者基本背景情報、Dファイル、EF統合ファイル、Hファイルの情報、入院総医療費に関する情報を取得する。 その後、カルテNoを使用してICU入室中のせん妄発症の有無に関する情報およびICU入室期間に関する情報を電子カルテより収集する。	We will list patients who meet the conditions described in the "Patients to be surveyed" section at the medical affairs section of the hospital, and obtain information on the patient's basic background, D file, EF integration file, H file, and total hospitalization cost from the medical record number and DPC data. Then, information on the onset of delirium during ICU stay and the duration of ICU stay are collected from the electronic medical record using the medical record number.
