目的1/Narrative objectives1

　超高齢化社会である日本において、高齢者の身体および認知機能の維持は喫緊の課題である。サービス付き高齢者住宅においても、身体および認知機能の低下によって、退去する利用者が存在し、その有効な予防プログラムの提供と内容が重要である。近年、運動療法が高齢者の身体および認知機能維持に有効である知見が蓄積されつつあるが、サ高住での効果については未だ明らかではない。また、当法人のサ高住「あやとり」では希望者に個別運動プログラムを実施しているが、運動プログラムが実際に個々の身体および認知機能に及ぼす効果は不明である。本研究の目的は、「あやとり」において個別運動プログラムが身体機能および認知機能に及ぼす影響を明らかにすることである。本研究の意義は以下の３つである。１）サ高住での個別運動プログラム提供が身体および認知機能に及ぼす影響を明らかにする、２）「あやとり」での個別運動プログラム向上のための基礎となるデータを集積する、３）「あやとり」での個別運動プログラムの有用性を示すことで、入居者の運動意欲を高め、他の高齢者施設との差別化を図る。

In super-aging society of Japan, the maintenance of physical and cognitive functions of the elderly is an urgent issue. The provision and content of effective preventive programs are important for elderly people in serviced housing for the elderly because some of them leave due to physical and cognitive decline. In recent years, knowledge on the effectiveness of exercise therapy in maintaining physical and cognitive function in the elderly has been accumulating, but the effectiveness of exercise therapy in the senior citizen population is still unclear. In addition, although we provide individual exercise programs to those who wish to participate in our "Ayatori" senior citizen residence, the actual effects of the exercise program on individual physical and cognitive function are unknown. The purpose of this study is to clarify the effects of the individualized exercise program on physical and cognitive function in "Ayatori". The significance of this study is threefold: 1) to clarify the effects of the Individualized Exercise Program on physical and cognitive function in the Ayatori, 2) to accumulate basic data to improve the Individualized Exercise Program in Ayatori, and 3) to demonstrate the usefulness of the Individualized Exercise Program in Ayatori, which will be used to improve the physical and cognitive function of the residents s motivation to exercise and differentiate themselves from other senior citizen facilities.

① 歩行評価
・　5mの距離を通常歩く速度と、自身の最大努力による速度で歩行する
・　歩行画像撮影（歩容評価）
・　足部装着型慣性センサー（歩行の時空間的評価）

副次アウトカム評価項目/Key secondary outcomes

② 運動器不安定性評価
・　Time Up & Go Test (TUG)：椅子から立ち上がり、無理のない早さで歩き、3m先の目印で折り返し、元の座った姿勢に戻るまでの時間を計測する
・　歩行評価と同様に足部装着型センサーを使用する
③ 筋力評価
・ 椅子立ち座りテスト：立った姿勢から連続して座り、立つ動作を10往復し、要した時間を計測する
・ 握力測定
④ 認知機能評価
・ Mini-Mental State Examination (MMSE)：時間の見当識、場所の見当識、3単語の即時再生と遅延再生、計算、物品呼称、文章復唱、3段階の口頭命令、書字命令、文章書字、図形模写の計11項目から構成される30点満点の認知機能検査（所要時間 6-10分）
・ Instruction manual of Japanese version of Montreal Cognitive Assessment (Moca-J)：視空間・遂行機能、命名、記憶、注意力、復唱、語想起、抽象概念、遅延再生、見当識からなる軽度認知症スクリーニング検査（所要時間 10分）

⑤ 心理状態評価
・ The Center for Epidemiologic Studies Depression Scale (CES-D)：20項目からなるうつ状態のスクリーニング検査

⑥ 日常生活評価
・ Barthel Index (BI)：食事、移乗、整容、トイレ動作、入浴、移動、階段昇降、更衣,排便自制、排尿自制の10項目を、それぞれ自立、部分介助など数段階の自立度で評価する

⑦ 研究対象者基本情報取得
・ 年齢、性別、身長、体重、足のサイズ、等
・ 「あやとり」内の運動教室、サークル活動の利用状況：運動教室や各種サークルへの参加の有無

「個別運動プログラム」は、PTと健康運動指導士が、入居者に対して、個別に運動プログラムを提案・指導し、入居者1人あたり約30分の運動を週1～2回実施する。
