選択基準/Key inclusion criteria	新型コロナウイルス感染症（COVID-19）と診断され（PCR、抗原検査）、研究協力機関に入院し退院した患者	Patients admitted to a cooperating hospital with a diagnosis of COVID-19 by PCR or antigen testing and discharged
除外基準/Key exclusion criteria	18歳未満、または医師が不適格と判断した者	Patients under the age of 18 Cases judged by the attending physician to be ineligible

その他関連情報/Other related information	前向き及び後ろ向き観察研究。新型コロナウイルス感染症（COVID-19）と診断され、研究協力施設に入院し、退院した症例を対象として、診療録から患者情報（年齢、性別、既往歴、COVID-19の治療状況、画像検査など）を抽出する。また、対象者に診断後3か月、6か月、12か月の時点でのCOVID-19の合併症・後遺症に関連するアンケート調査を行う。	Prospective and retrospective observational study. Patient information (age, gender, past medical history, treatment for COVID-19, imaging studies, etc) will be extracted from the subject's medical records for patients aged 18 years or older who were diagnosed with COVID-19, admitted to a cooperating institution, and discharged. In addition, subjects will be asked to complete questionnaires related to complications and sequelae of COVID-19 at 3, 6 and 12 months after diagnosis.
