腸管内容量や局在に関して腹部シネMRIで視覚的・客観的評価を行い、腹部症状との関連を検討する.健常者と腹部症状のある患者における検査食前後の腸管内容量・腸管径・蠕動運動を比較検討する

To perform visual and objective evaluation of intestinal volume and localization using abdominal cine MRI and to examine the relationship with abdominal symptoms.
To compare the intestinal volume, intestinal diameter, and peristalsis before and after the test meal in healthy subjects and patients with abdominal symptoms.

選択基準/Key inclusion criteria

日本語
器質的疾患が除外されている腹部症状のある患者、上部内視鏡検査あるいは胃透視を施行し器質的疾患が除外されている機能性ディスペプシア患者・下部内視鏡検査あるいはバリウム注腸検査を施行し、器質的疾患が除外されている慢性便秘症の患者
腹部症状のない健常者。
本研究への参加について書面によるインフォームド・コンセントが得られている。

Patients with abdominal symptoms (functional dyspepsia, chronic constipation) who have undergone Upper endoscopy or upper gastrointestinal barium examination for functional dyspepsia / lower endoscopy or barium enema examination for chronic constipation and are excluded from organic diseases.

or healthy subjects without abdominal symptoms.

Written informed consent provided for study participation

除外基準/Key exclusion criteria

① 器質性便秘（大腸癌、炎症性腸疾患、巨大結腸）の患者
② 腸切除後の患者
③ 進行胃癌・進行大腸癌の患者
④ 活動性消化性潰瘍の患者
⑤ ペースメーカー、人工関節などMRI非対応の金属の使用や刺青を施行している患者、
閉所恐怖症の患者
⑥ 検査食にアレルギーのある患者
⑦ その他、研究責任者が研究対象者として不適当と判断した患者

1)Organic constipation(colorectal cancer, inflammatory bowel disease, megacolon)
2)After gastrointestinal tract resection.
3)Advanced gastric cancer, advanced colorectal cancer
4)Active peptic ulcer
5)Non-MRI compatible metals such as pacemakers and artificial joints or who are undergoing tattooing
Patients with claustrophobia
6)Patients who are allergic to test meal
7) Considered by the principal investigator to be ineligible.

主な結果/Results

日本語
食前と食後のHC群とFD群の比較結果として、食前では、収縮頻度を除くどのパラメーターにも有意差は認めなかった (HC: 1.4 ± 1.2/FD: 1.6 ± 1.3 回/分、P < 0.001)。しかし、食後では、前庭部の最大短軸径 (34.8 ± 8.5/37.5 ± 10.8 mm、P < 0.001)、振幅 (7.3 ± 5.1/11.9 ± 5.8 mm、P < 0.001)、収縮頻度 (2.9 ± 0.3/2.7 ± 0.5 回/分、P = 0.002)、蠕動波高 (14.9 ± 4.0/9.3 ± 2.7 mm、P < 0.001)、および GMI (24.5 ± 7.1/16.6 ± 6.1 mm2/s、 P < 0.001)で有意差が認められた。

The results of the comparison between the HC and FD groups in the preprandial and postprandial periods; In the preprandial period, there was no significant between-group difference in any parameter except contraction frequency. However, in the postprandial period, there were significant differences in the maximum short axis diameter of the antrum , amplitude of the fornix, contraction frequency, peristaltic wave height, and GMI.

8時間前より絶食とし、シネMRIは、試験食品摂取前と摂取後5分後に実施された。
シネ MRI 画像は、摂取中に仰臥位で取得され、息止め画像は 20 秒間隔でそれぞれ 20 秒間 2 回実施された。

Participants were fasted for 8 hours before cine MRI. Cine MRI was performed before and 5 minutes after test food ingestion.
Cine MRI images were acquired in the supine position during ingestion, with breath-hold imaging conducted twice for 20 seconds each, separated by a 20-second interval.
