移行椎に関する認識度を向上させ、手術高位の間違えや手技上の間違えを減らすことが本研究の目的である。また研究の詳細な情報は、脊椎外科医、放射線科医などに有益である。

The aim of this study was to increase the awareness of TLTV and LSTV to reduce wrong-site surgery or diagnostic errors. Detailed knowledge of these transitional vertebrae may potentially benefit spine surgeons as well as radiologists and interventional pain medicine specialists.

胸腰椎移行部および腰仙椎移行部における移行椎が存在することで、手術高位を間違えることがあるが、脊椎骨盤パラメーターの計測にも問題を生じさせる可能性がある。Castellviの腰仙椎移行椎の分類では、その形態学的特徴は十分には把握できない。したがって、腰仙椎移行椎の形態を調査するためにCTを用いた。

The presence of transitional vertebra in the thoracolumbar and/or lumbosacral regions may cause wrong-site surgery and problems while measuring spinopelvic parameters, including pelvic incidence and lumbar lordosis. The Castellvi classification of lumbosacral transitional vertebra (LSTV) addresses coronal images but not sagittal or axial images. Therefore, it is unclear how LSTV differs from the normal lumbosacral anatomy. We aimed to investigate the lumbosacral anatomy and vertebral numbering in patients with thoracolumbar transitional vertebrae (TLTV) and/or LSTV by performing computed tomography (CT) to identify TLTVs, to number presacral vertebrae accurately and to analyze morphological differences in each LSTV type.

LSTVは880人の脊椎固定術を受けた患者のうち111人 (12.6%)に認め、Castellvi type IIIが最も多く、111人中42人 (37.8%)、次いで、S6 typeが37人 (33.3%)であった。TLTVは87人に認め、LSTVと高率に並存し、87人中59人はT13に認めた。

LSTVs were observed in 111/880 patients (12.6%). Castellvi type III LSTVs were the most common (42/111, 37.8%), followed by S6 LSTVs (37/111, 33.3%). TLTVs were associated with LSTVs (87/111, 78.4%) and commonly identified at T13 (59/87, 67.8%).

2014年7月から2020年3月の期間に脊椎固定術を受けた患者880人を、TLTVとLSTVについて評価した。本研究では、880名の患者のうち124名（14.1％）に移行椎または椎体数の異常が見られた。この124名の患者のうち60.5％を男性患者が占めていて、平均年齢（および標準偏差）は69.7±12.0歳であった。

The medical records of 880 patients who underwent thoracolumbar fusion between July 2014 and March 2020 were evaluated for TLTV and LSTV. In this study, 124 (14.1%) of 880 patients had transitional vertebrae and/or vertebral numeric variation, with a mean age (and standard deviation) of 69.7 (12.0) years and male patients accounting for 60.5% of these 124 patients.
