自宅での簡易PSGにSmart WatchないしWatch PATを使用した患者

除外基準/Key exclusion criteria

日本語
検査の全経過でセンサーの脱落等により記録の脱落部分がある記録。
簡易PSGの総検査時間が280分未満の記録
標準PSGでの総睡眠時間が240分未満の記録。

在宅検査で仰臥位RDIは29.3,非仰臥位は11.3/hr、入院標準検査の仰臥位AHIは52.0,非仰臥位は25.2/hrで共に体位の影響が強かった。在宅検査時間の70%以上が非仰臥位だった患者でAHI=2.42×RDI-8.02, r=0.67が得られた。残りの患者を仰臥位と非仰臥位のRDI比0.47で側臥位改善群と非改善群に分割し、補正RDI=仰臥位RDI×0.491+非仰臥位RDI×0.509の式に代入すると、改善群でAHI=1.05×補正RDI+17.8, r=0.58、非改善群でAHI=1.17×補正RDI +13.0, r=0.45が得られた。

RDI and AHI were strongly affected by body position. In those whose supine time <30% of total sleep time, AHI was predicted as 2.42xRDI-8.02, r=0.67. Remaining patients were classified as position dependent (RDI ratio between supine and other positions > 0.47) and independent. Using corrected RDI (RDI at supinex0.491+RDI at other positionsx0.509), predicted AHIs were 1.05xcorrected RDI+17.8, r=0.58 in position dependent group and 1.17xcorrected RDI+13.0, r=0.45 in independent one.

カルテ記載から①患者背景(年齢、性別、身長、体重)、②病歴情報(原疾患、合併症、飲酒歴、JESSによる眠さ尺度、睡眠時間）、③簡易PSGの検査値(無呼吸低呼吸指数:AHI、仰臥位AHI、側臥位AHI、無呼吸指数AI、仰臥位時間、側臥位時間、実睡眠時間、装着時間)、④標準PSGの検査値（AHI、仰臥位AHI、側臥位AHI、無呼吸指数、仰臥位時間、側臥位時間、就床時間）を取得する。
以下の比較検討を行う。1)同一患者での標準PSG(アリス)と簡易PSG(スマートウォッチ)の比較:無呼吸指数、仰臥位時間、側臥位時間)。2) 同一患者での標準PSG(アリス)と簡易PSG(ウオッチパット)の比較:実睡眠時間、装着時間。3)3)異なる患者での標準PSGアリスとNOX A1システムの比較。
統計解析処理（多変量解析等）を実施
主要評価項目
同一患者での標準PSG(アリス)によるAIを目的変数とし、簡易PSG(スマートウォッチ)によるAIを説明変数とする。仰臥位時間優位と側臥位時間優位に群分けして、共分散分析を行う。
副次的評価項目
異なる患者において、新規の標準PSG(NOX A1システム)によるAIを目的変数とし、従来型の標準PSG(アリス)によるAIを説明変数とする。仰臥位時間優位と側臥位時間優位に群分けして、共分散分析を行う。

Following information is obtained from clinical records. 1.Patients' background (age, gender, height, weight). 2.Clinical information (name of disease, coexisting illness, alcohol history, JESS, sleeping time). 3Results of home study (total apnea hypopnena index-AHI, AHI at lateral position, AHI at supine position, apnea index, supine time, lateral time, true sleeping time, study time). 4Results of laboratory study (total AHI, AHI at lateral position, AHI at supine position, apnea index, supine time, lateral time, true sleeping time, study time).
Compare results on level 1 polysomnography (PSG) in laboratory and level 3 PSG in home (AHI, supine time lateral time). Two types of level 3 PSGs, i.e., Smart watch and Watch PAT, are used. Results of two type of level 1 PSGs, i.e., Alice and NOX1, are also examined.
multivariate analysis will be applied for statistical analyses.
