目的/Objectives
目的1/Narrative objectives1	体外受精・胚移植にあたっては、いかに着床能の高い高品質の受精卵を選別し妊娠率を向上させるかが重要である。現在、受精卵の品質の評価は形態のみで行われているが、形態学的評価法は主観性が強く観察者間での結果に差が生じてしまう。我々は、これまでに受精卵の呼吸機能と卵品質との間に関連性が認められる可能性に着目し、その有用性を報告してきた(平成25年度~平成26年度厚生労働省科学研究費補助金「受精卵故宮測定装置を用いた臨床研究に橋渡しするための安全性および有用性に関する研究」)。この手法は非 常に高感度である上に侵襲もない画期的な装置と考えられる。受精卵呼吸測定装置のプロトタイプは平成25 年度~平成26年度厚生労働省科学研究費補助金「受精卵故宮測定装置を用いた臨床研究に橋渡しするための安全性および有用性に関する研究」における共同研究者らが開発し、クリノ社が既に細胞呼吸測定機器 (製品名CRAS1.0、CRAS3.0) として販売している。これまでに動物卵を用いて、その受精卵呼吸測定装置の有用性と安全性を報告している。また、ヒト臨床検体においても、安全性および有効性が国内医療機関から報告されている。CRAS3.0は、煩雑な操作なしに受精卵の呼吸量を測定でき、CRAS1.0では困難であった操作の単純化を満たしたものである。本器は受精卵に触れることはなく、使用する電流も微弱なため有害事象の発生する可能性も極めて低い。本研究は、受精卵の機能的評価法として、優良卵の選別に有用な臨床 検査法を提供できる。	To clarify whether embryo respiration reflect the quality and viability of the embryo or not.in clinical use.

主要アウトカム評価項目/Primary outcomes	胚移植後の血中hCG陽性率、着床率、臨床的妊娠率、継続妊娠率、流産率、早産率、生産率および出生児 の性別、出生体重、身長	Pregnancy rate, Implantation rate, Clinical pregnancy rate, Ongoing pregnancy rate, Abortion rate, Preterm labor rate, Sex, height, body weight of newborn.

選択基準/Key inclusion criteria	年齢≦45才 卵巣の反応が正常であると考えられる者(胞状卵胞数(AFC)が4個以上20個以下、FSH≦10mIU/ml、AMH> 1) 受精卵数が4個以上 高度な子宮因子、卵管水腫、子宮内膜症がない	Age: under 45 years old AFC: 4 or more, FSH: less than 10, AMH: 1 or more Fertilized embryo: 4 or more Without uterine factor, hydrosalpinx or pelvic endometriosis
除外基準/Key exclusion criteria	遺伝性疾病がある 卵巣因子及び卵管因子による不妊症 子宮因子による不妊	Any hereditary disease Ovarian or tubal infertility Uterine infertility
