バンクーバー瘢痕スケールで術後の評価を行い、術後１、２、４週後と６ヶ月後に写真を撮る

年齢は20歳から94歳（平均年齢61歳）、男性30人、女性10人、植皮術が適応となった原疾患は熱傷19人、難治性潰瘍12人、腫瘍切除後皮膚欠損創4人、皮弁採取部2人、デグロービング2人、皮下血腫2人、瘢痕拘縮2人、足壊疽1人、フルニエ壊疽2人であった。
基礎疾患は糖尿病13人、高血圧12人であった。

The patients ranged in age from 20 to 94 (mean age 61), with 30 men and 10 women. The primary diseases for which skin grafting was indicated were burns in 19 patients, intractable ulcers in 12, skin defects after tumor resection in 4, skin flap donor sites in 2, degloving in 2, subcutaneous hematoma in 2, scar contracture in 2, foot gangrene in 1, and Fournier's gangrene in 2.
Underlying diseases included diabetes in 13 patients and hypertension in 12.

2020年4月から2023年9月までに川崎医科大学付属病院形成外科において20歳以上で分層植皮が適応となった症例を対象とした。
大腿部から採皮した症例のみを抽出し、経過中に他院への転院した症例や自己中断により術後6カ月時に診察できなかった18例は除外し、21例を対象として経過観察を行った。

The study included patients aged 20 years or older who underwent split-thickness skin grafting at the Department of Plastic Surgery, Kawasaki Medical School Hospital, between April 2020 and September 2023.
Only cases in which skin was harvested from the thigh were extracted, and 18 cases who were transferred to other hospitals during the follow-up period or who were unable to be seen 6 months after surgery due to self-interruption were excluded, resulting in a follow-up observation of 21 cases.

Control群で1例の感染によるびらん形成と上皮化遅延があったが、他の症例では感染などの明らかな合併症を認めなかった。
VSSについてはBack graft群では中央値2.5(最小値０、最大値5)、control群では4.5(最小値1、最大値8)、p<0.05となり有意差を認めた。
機械を用いた色調の測定は色相と彩度を示すa*b*はback graft 群では中央値4.45(最小値2.90、最大値7.05)、control群では中央値8.57(最小値3.12、最大値113.38)、p<0.05であり有意差を認め、back graft群の方が有意に健常部の色調に近い瘢痕となった。 明るさを示すL*では単純にL*軸方向の距離のみでの検討を行った。 採皮部と健常部の2点間の距離はback graft 群では中央値6.70(最小値0.86、最大値7.05)、control群では中央値15.56(最小値3.70、最大値22.12)、p<0.05であり有意差を認め明るさにおいてもback graft群の方が健常部に近い瘢痕となった。
ミンチ状戻し植皮により採皮部瘢痕の良好な経過を認めた。

The number of days to epithelialization, VSS, and color evaluation using a spectrophotometer were performed.
The median time was 9.5 days (minimum 7, maximum 12) in the back graft group and 20.0 days (minimum 8, maximum 32) in the control group, with a significant difference of p<0.05.
One case in the control group involved erosion and delayed epithelialization due to infection, but no other cases showed any obvious complications such as infection.
The median VSS was 2.5 (minimum 0, maximum 5) in the back graft group and 4.5 (minimum 1, maximum 8) in the control group, with a significant difference of p<0.05.
Machine-based color measurements of a* and b*, which indicate hue and saturation, showed a median of 4.45 (minimum 2.90, maximum 7.05) in the back graft group and a median of 8.57 (minimum 3.12, maximum 113.38) in the control group, p<0.05, showing a significant difference, indicating that the back graft group had scars significantly closer to the color of the healthy area. For L*, which indicates brightness, we simply examined the distance along the L* axis. The distance between two points on the donor site and the healthy area was a median of 6.70 (minimum 0.86, maximum 7.05) in the back graft group and a median of 15.56 (minimum 3.70, maximum 22.12) in the control group, p<0.05, showing a significant difference, indicating that the back graft group also had scars closer to the healthy area in brightness.
Minced skin grafting demonstrated favorable progress in the donor site scar.
