目標とする生活行為への介入（残存する工程や認知機能の活用，技能練習，環境介入，家族支援・教育）を1回/週、1回40分、3か月間実施する．

Participants will be received to combined interventions for ADL independence (use of remaining cognitive function, skill training, environment support, support and education for families, etc.) for 40 minute, once a week, for 3 months.

主な結果/Results

1．ベースラインの比較：対象者は，COVID-19関連を含むドロップアウトを除外して，最終的に介入群35名，対照群28名を分析対象とした．ベースラインでの2群間比較については，基礎的情報，認知機能など全てにおいて有意差なく，同等の対象条件であった．
2．介入前後比較：Lawton IADL，PADA-D総合得点，IADLに有意な交互作用が見られ，介入効果が認められた．認知機能，行動心理症状，他ADL尺度には有意な変化なかった．行為別では洗濯に交互作用が認められた・
3．目標とした生活行為の満足度と遂行度：介入後，満足度，遂行度共に有意に向上した．

Total and instrumental/basic activities of daily living (ADL) Process Analysis of Daily Activity for Dementia scores showed significant interaction effects, and performance and processes related to Laundry improved. Rehabilitation strategies, including repetitive skills practice, were combined according to the goals.

参加者背景/Baseline Characteristics

参加者は、2019年から2022年の間に、日本国内の13の医療・介護施設（認知症専門医療施設、認知症専門外来クリニック、デイケア・リハビリテーション施設、訪問看護ステーションを含む）から募集された．選択基準(1) 認知症専門医によりDSM-5-TR診断基準[2]に基づいてアルツハイマー病（AD）による主要な神経認知障害と診断されている，(2) 65歳以上の高齢者，(3) MMSEスコアが10点以上，(4) 自宅で生活している，(5) 生活をよく知る家族がいること，とした．

Participants were recruited between 2019 and 2022 from 13 medical and care centers in Japan, including medical centers for dementia, outpatient clinics specializing in dementia, day care and rehabilitation centers, and visiting nurse stations. The selection criteria were as follows: (1) major neurocognitive disorder caused by AD diagnosed by a dementia specialist based on the DSM-5-TR diagnostic criteria [2], (2) age 65 years or older, (3) Mini-Mental State Examination (MMSE) score >10, (4) living at home, and (5) family members familiar with the living situation.
