評価/Assessment
主要アウトカム評価項目/Primary outcomes	ゲノム解析によるレアバリアント、単一塩基遺伝子多型（SNP）、メタボローム解析(代謝産物の網羅的解析)によるメタボライト(代謝産物)、プロテオーム解析(タンパク質の網羅的解析)におけるタンパク質、およびマイクロRNA。	Genetic rare variant, single nucleotide polymorphism, metabolite, protein, and micro RNA.
副次アウトカム評価項目/Key secondary outcomes	臨床情報　(性別、年齢、身長、体重、腹囲、BMI、血圧、心拍数、身体活動能力指数、NYHA、Stage分類、心疾患、心エコー、血液学検査、免疫血清検査、尿検査、尿アルブミンクレアチニン換算値検査、生化学検査、胸部レントゲン、12誘導心電図、既往症(心不全入院歴、高血圧歴、糖尿病歴、高脂血症歴、高尿酸血症歴、腎不全、透析、脳血管疾患、心房細動、心室頻拍、癌)、薬剤過敏症、喫煙歴、飲酒歴、心疾患家族歴)。	Clinical information (sex, age, height, body weight, blood pressure, heart rate, SAS, NYHA class, HF stage class, heart disease, echocardiogram, blood test, urine test, chest X-ray, electrocardiogram, drug allergy, past history, smoking, drinking, family history).

選択基準/Key inclusion criteria	東北大学病院循環器内科外来に通院中の慢性心不全およびそのハイリスク患者。	Patients with or at high risk of chronic heart failure those attending Department of Cardiovascular Medicine, Tohoku University Hospital.
除外基準/Key exclusion criteria	担当医師が本研究の対象として好ましくないと判断した患者。	Patients who were judged to be not eligible in the present study by the physician in charge.
