除外基準/Key exclusion criteria	①未成年の患者、妊娠している患者 ②自らの意思で研究の同意が出来ない患者 ③その他医師が症例組み入れに不適当と判断した患者	1.age below 20 years old or pregnant 2.those who cannot agree to the study 3.inappropriate patients judged by primary or study physicians.
