中下部胆管閉塞を有する切除不能膵癌に対する標準的胆道内瘻術である10mm径MSに対して、14mm径MSによる開存期間延長に関する優越性を検証すること。

選択基準/Key inclusion criteria
日本語
(1) 黄疸または肝機能異常を有する
(2) 悪性の病理組織学的確証が得られた膵臓癌患者
(3) 切除不能
(4) 拡張胆管径が14mm以上である
(5) 胆管狭窄が肝門部に達しない中下部に存在する
(6) ECOG Performance Status (PS) 0～1の患者。
(7) 4ヶ月以上の生存期間が期待できる
英語
(1) Patients with jaundice or liver dysfunction
(2) Pathologically confirmed as cancer
(3) No indication for surgical resection
(4) The diameter of dilated bile duct >= 14mm
(5) Distal bile duct obstruction which is not reached in the hilar region
(6) Performance status(ECOG) is 0~1.
(7) Prognosis will be >= 4 months.
除外基準/Key exclusion criteria
日本語
(1) 胃切除後でBilloth-I再建以外の再建法
(2) 十二指腸乳頭部の肛門側腸管閉塞患者。
(3) 過去に金属ステントが留置された患者。
(4) 他臓器に重篤な合併症を有する症例(ASA分類の3以上。)
(5) 急性膵炎、慢性膵炎急性増悪併存患者。
(6) 肝硬変患者
(7) 血小板数が50,000/mm3以下である。
(8) 主治医が本試験を実施するのに不適当と認めた患者。

(1) Patients with history of gastrectomy and reconstruction other than Billroth I anastomosis
(2) Duodenal obstruction in anal side of major papilla
(3) Patients with history of insertion of biliary metallic stent
(4) ASA >= 3
(5) Patients with acute pancreatitis or exacerbation of chronic pancreatitis
(6) Patients with liver cirrhosis
(7) Patients with Platelet count=< 50,000 mm3
(8) A participating doctor thinks the patient is inappropriate for this study.
