落屑緑内障は眼組織が落屑物質を産生し、水晶体、虹彩などに進行しに沈着することを特徴とする。落屑物質にはグリコサミノグリカンの存在が示唆されており、その過剰産生や異常代謝が原因の一つと考えられている。落屑物質は水晶体前面、瞳孔縁、チン小帯、角膜内皮、隅角などに沈着し、瞳孔運動による水晶体表面の研磨により落屑物質が散布され、隅角の線維柱帯がつまることにより緑内障が発生すると考えられている1）。
これまで落屑のみ伴う症例と落屑緑内障の眼内の落屑濃度が比較された報告はなく、緑内障への移行率は不明である。また、過去に落屑緑内障において線維柱帯切開術が効果的であるとされる報告が散見されるが2）3）、ここ最近術後5年目で眼圧が上昇してくる症例がみうけられ、落屑濃度が術後成績に関与している可能性が推測される。
　今回落屑緑内障の術後成績に関与する因子として前房内落屑濃度に注目した。術中に前房水を採取しその濃度を測定することで落屑緑内障の発生率の関与、および術後成績の関連を評価し、その他の所見との相関を検討する。

There is no report comparing desquamation concentrations in the eye of desquamation glaucoma with cases with only desquamation, the rate of transition to glaucoma is unknown. In addition, reports showing that trabeculectomy is effective for desquamation glaucoma in the past have been reported 2) 3), in recent years there have been cases in which intraocular pressure rises in the fifth year after surgery , It is inferred that the desquamation concentration may be involved in postoperative results.
We focused on the concentration of desquamation in the anterior chamber as a factor related to the postoperative outcome of desquamating glaucoma. By collecting the anterior chamber water during surgery and measuring its concentration, the involvement of the incidence of desquamation glaucoma and the association of postoperative results are evaluated and the correlation with other findings is examined.

白内障以外の疾患を認めない白内障手術群、落屑を認めるが緑内障を発症していない白内障手術群、落屑緑内障を認める緑内障手術群の3群で術前に説明を行い承諾を得た症例

白内障および緑内障以外の疾患を持っている患者。前房水採取が困難である患者。固視不良の患者。
