適格性/Eligibility
年齢（下限）/Age-lower limit	20 歳/years-old 以上/<=
年齢（上限）/Age-upper limit	99 歳/years-old 以下/>=
性別/Gender	男女両方/Male and Female
選択基準/Key inclusion criteria	1)　同意取得時の年齢が20歳以上の患者 2)　慢性心不全ステージC・D患者 3) 心不全増悪にて入院した患者　 4)　本人から文書による同意が得られた患者	1) Age: 20 years or more 2) Stage C or D chronic heart failure 3) Admitted to hospital due to heart failure exacerbation 4) Written informed consent obtained
除外基準/Key exclusion criteria	1)　意思疎通ができず、アンケートの内容を理解・回答できない患者・家族 2)　痛み、呼吸困難、倦怠感、吐き気・嘔吐などの身体症状が重篤で、担当医 が研究の参加に妥当でないと判断した患者 3)　精神症状が重篤で、担当医が研究の参加に妥当でないと判断した患者 4)　移植ネットワーク登録患者 5)　説明・同意文書の内容を理解できない患者・家族 6)　他の臨床試験に既に参加もしくは本試験期間中に参加する予定がある患者 7)　その他の理由により、試験責任医師または分担医師が本試験への参加を不適当と判断した患者	1) Communication difficulty 2) Serious physical symptoms 3) Serious psychological symptoms 4) Registered in heart transplant network 5) Difficulty in understanding explanation of the study 6) Enrollment in other clinical study 7) Inadequate for participating the study by any reason judged by the attending physician
