日本語
1.糖尿病患者でフットケア外来を受診している者
2.日本語でコミュニケーションが取れる
3.糖尿病足病変の悪化の可能性があることを知っている
4.自らの足を観察できる視力があり、フットケアを実施することができる身体機能を有する。もしくは、これらを代替する家族の支援を得ることができる。
5.重篤な精神疾患を合併していない

1.Patient is a diabetic patient who is visiting a foot care outpatient
2.Patient can communicate in Japanese
3.Patient knows diabetic foot
4.Patient can observe his or her feet by himself and has a physical function capable of performing foot care or their family can support them
5.Patient doesn't have serious cognitive disorder and mental disease

除外基準/Key exclusion criteria

日本語
身体的、精神的、認知的理由で質問紙調査・プログラム実施に耐えられないと主治医が判断した者

The primary doctor judged that patient can not participate in the questionnaire survey / program for physical, mental and cognitive reasons
