| 目的/Objectives | ||
| 目的1/Narrative objectives1 | 当病棟は血液内科の疾患が多く、長期的に治療を継続しなければならない症例が多い。そのために退院後の生活について入院早期より考え、計画立てた指導が必要である。また長期間の治療が必要であるために医師に対する患者の信頼も強い。したがって、医師・看護師の連携も不可欠である。本研究の目的は、退院支援に対する医師・看護師の意識調査から血液内科における退院支援を円滑にすすめるための退院支援ツールを作成・導入し、その効果を検討することである。 | This study is aimed to develop the new system supporting early discharge from hospital for the patiens with hematological disorder. |

| 主要アウトカム評価項目/Primary outcomes | 1）対象者に研究の目的と方法を文書を用いて説明し、書面にて同意を得た後に、質問紙調査（資料1）を実施する(質問紙調査1)。
2）質問紙調査1から得られた結果は頻度集計、クロス集計を行う。 3）対象者のうち、研究者らで抽出した、臨床経験5年以上で退院支援の経験のある看護師3名、血液内科医3名の計6名を対象として、研究の目的と方法を文書を用いて説明し、書面にて同意を得た後に、インタビューガイド（資料2）を用いたグループインタビュー（グループインタビュー1）を行う。インタビュー内容は同意を得た上で録音する。 4) インタビュー内容の逐語録を作成し共同研究者5名でカテゴリー別に分類、統合分析後 メンバーチェッキングを行う（資料3）。 5) 質問紙調査1、グループインタビュー1の結果を参考にしながら退院支援ツールを作成する（原案:資料4-1、4-2）。 6) 退院支援ツールをB8号病棟において使用する(期間は約6か月)。 7) 再度、対象者に質問紙調査（資料1）を実施する（質問紙調査2）。 8) 質問紙調査2から得られた結果は頻度集計、クロス集計を行い、質問紙調査1と比較する。 9) 再度、対象者にグループインタビュー（資料2）を行う（グループインタビュー2）。 10) グループインタビュー2の分析結果は、グループインタビュー1の分析結果と比較する。 11) 8)と10)の結果から導入した退院支援ツールの効果について検討する。 |
Primary outcome is the number of hospitalization days and degree of satisfaction with or without newly developed discharge support system. |

| 選択基準/Key inclusion criteria | B8号病棟で実際に退院支援を実施するときに、患者、家族に関わる医師、看護師に対して実態調査を行う事で問題点や改善策を見出し協同して実際に支援を行うことができるため、対象者をB8号病棟に所属する看護師・医師とする。 | Because I find a problem and a reform measure and cooperate, and it's possible to support it with conducting surveys on it to a patient, a doctor of the family and a nurse actually when conducting leaving support in a ward actually. | |||
