上部消化管病変に対する内視鏡的粘膜下層剥離術前の口腔ケアが誤嚥性肺炎の予防に有効であるか検討する。同時にランダム割付された口腔ケアをしない群との比較検討を行う。

選択基準/Key inclusion criteria

1) Stage IAの早期胃癌(疑い)もしくは胃腺腫(疑い)と診断されている。
2) 登録時20歳以上である。
3) 試験参加に関して患者本人から同意を得ている。

1) Clinical diagnosis of Stage IA early gastric cancer or gastric adenoma, and suspicion for Stage IA early gastric cancer or gastric adenoma.
2) 20 years old or older.

主な結果/Results

日本語
口腔ケアは誤嚥性肺炎を予防しなかったが、肺炎発症にペチジン塩酸塩の使用量や脳血管疾患 既往などの基礎疾患が寄与していることが明らかになった。

Sixty-one patients who underwent ESD for gastric neoplasms were randomly assigned to the oral care or control groups. Of 60 patients whose data were available for analysis, 5 (8.3%) experienced pneumonia . The post-oral care bacteria count was significantly higher in the saliva of patients who developed pneumonia compared to those without pneumonia. In addition, the presence of vascular brain diseases and the dose of meperidine were also significantly associated with the occurrence of pneumonia.
