覚醒下手術は脳高次機能を温存した手術を可能とする．保険収載されるようになったことから（頭蓋内腫瘍摘出術［脳腫瘍覚醒下マッピング加算］），本邦でも数多くの施設が覚醒下開頭手術を手がけるようになってきた．しかしながら，その術中管理法は通常の全身麻酔症例とは大きく異なる．気道確保が不確実であり，嘔吐やけいれんなどの急変リスクを伴うだけではなく，手術目的を達成しうるだけの明瞭な覚醒状態を長時間維持することが求められるからだ．当施設においても1999年の初回症例以降，その術中管理法や使用薬剤には工夫が重ねられてきた．
本研究では，覚醒下手術をより安全におこなうことを目的として，その周術期管理法について後ろ向きに調査する．当施設はこの分野において世界でも有数の症例数を誇るが，これらの症例に関して検討することにより，今後施行される覚醒下開頭手術の“質”の向上に寄与し，情報を全世界で共有することができる．

An iMRI-guided neurosurgery with functional mapping under awake craniotomy contributes the maximal surgical resection with a minimal risk of postoperative deficits when a tumor is in or adjacent to the eloquent area. However, it could be challenging because of unsecured airways with risks of vomiting, epileptic attacks or unstable levels of consciousness.
In this study, we reviewed anaesthetic charts, surgical dictations and clinical records for awake craniotomy for glioma resection to evaluate our strategy of preoperative management. Our study will contribute to improve quality of the patient management.
