目的/Objectives
目的1/Narrative objectives1	急性冠症候群の発症機序は、冠動脈不安定プラークの破綻と血栓形成と考えられている。また、不安定プラークに対して経皮的冠動脈インターベンション (PCI) を行う際には、末梢塞栓や再還流障害を起こしやすいことが知られている。しかし、このような不安定プラークを予測可能な検査方法は確立されていない。本研究の目的は、不安定プラークを非侵襲的に予測可能なバイオマーカーを確立することである。	Rupture of vulnerable plaque (VP) with thrombus formation is the main mechanism of acute coronary syndrome. Furthermore, percutaneous coronary intervention (PCI) to VP is associated with distal embolism and reperfusion injury. However, the methods predicting the presence of VP have not been established. The aim of this study is to determine the useful biomarkers to predict VP existence.

選択基準/Key inclusion criteria	心臓カテーテル検査あるいはPCIの目的で当院に入院した冠動脈疾患患者、あるいは冠動脈疾患が疑われる患者	Patients or suspected patients with coronary artery disease who admit to our hospital for cardiac catheterization or PCI
除外基準/Key exclusion criteria	1. 透析を行っていない高度の腎不全患者 2. 出血症状のある患者、出血性素因のある患者及び止血障害のある患者 3. 貧血を認める患者 4. 造影剤アレルギーのある患者 5. 心臓、血管内へのカテーテル挿入が困難、あるいは困難と予想される患者	1. Patients with end-stage renal disease without hemodialysi 2. Patients with bleeding risk or hemostatic disorder 3. Patients with anemia 4. Patients with contrast media allergy 5. Patients with the potential difficulty in manipulation of catheter

その他関連情報/Other related information	1. 冠動脈疾患の異なる病態間（安定狭心症、急性冠症候群、冠攣縮性狭心症など）におけるバイオマーカーの差異の検討 2. プラークの異なる性状間（線維性プラーク、脂質性プラーク、不安定プラーク、破綻したプラークなど）におけるバイオマーカーの差異の検討：血管内超音波（IVUS）や光干渉断層法（OCT）による評価 3. PCIの前後におけるバイオマーカーの変化の検討 4. PCI後心筋障害の有無によるバイオマーカーの差異の検討：高感度トロポニンTなどの心筋逸脱酵素による評価 5. 全身の血管内皮機能・動脈硬化の指標とバイオマーカーの対比	1. To evaluate the significance of biomarkers among different types of coronary artery diseases including stable angina pectoris and acute coronary syndrome. 2. To evaluate the significance of biomarkers among different characteristics of coronary plaques including fibrotic, lipidic, and vulnerable plaque observed by intracoronary imaging modality. 3. To evaluate the changes of values of biomarkers before and after PCI. 4. To evaluate the significance of biomarkers in patients with and without peri-procedual myocardial injury. 5. To compare the biomarkers with indices of endothelial function and atherosclerosis.
