目的/Objectives

日本語
本研究の主目的は、3テスラMRIを用いて、日本人のハイリスク頚動脈プラーク性状を解明することである。その為に、症候性プラークと無症候性プラークの性状について、横断的、縦断的に検討していく。
(1) 症候性、無症候性プラークの比較　（横断的研究）
日本人の頚動脈における症候性プラークと無症候性プラークを比較し、症候性プラークの特徴を明らかにする。複数のプラーク構成要素（プラーク内出血、脂質コア、石灰化、線維性被膜等）に着目して、形態的に包括的な検討をする。

(2) 脳虚血性イベントの予測因子となるプラーク性状の検討　（前向き研究）
頚動脈狭窄病変症例のプラークを撮像後、追跡調査を行い、将来イベントを生じる頚動脈プラークの特徴を明らかにする。

The main purpose of this study is to reveal high-risk carotid plaque characteristics for Japanese patients with carotid atherosclerotic disease using in vivo 3T MRI. First, we will compare carotid plaque characteristics between symptomatic and asymptomatic carotid stenosis. Second, we will follow up patients with asymptomatic carotid stenosis and examine the relationship between carotid plaque features and future celebrovascular ischemic events.

本研究の主目的は，日本人の頸部頸動脈動脈硬化性病変（頸動脈プラーク）の特徴を3テスラ MRIを用いて明らかにすることであった．
日本人と米国人の頸動脈プラーク性状を比較したところ，特定の患者群においてプラーク性状が異なる可能性が示唆された．また，日本人においても，脳虚血性疾患の原因と考えられた頸動脈プラークと，無症状のプラークで比較すると，前者のプラークの方が，有意にハイリスクプラークであることが分かった．
以上から，３テスラMRIを用いた，日本人における頸動脈プラークの評価は可能であり，症状と関連するプラーク性状の特徴を把握することができた
