目的1/Narrative objectives1

日本語
糖尿病や末梢動脈疾患に伴う難治性潰瘍症例では、現状維持に最低限の血流は維持されているが、いったん潰瘍が形成されると、治癒させるのに十分な酸素・栄養の供給が困難となり壊死が進行する。また栄養や抗生剤の運搬、白血球遊走が不十分であるため感染コントロールが困難となりやすい。手術療法を施行した場合も、移植床および移植組織ともに循環障害が存在し、皮弁に十分な血流が行き渡らず感染や壊死の可能性が高い。
酸素は代謝、マトリックス形成、感染コントロール、細胞増殖刺激などに関与すると考えられている。本研究では、難治性潰瘍やその皮弁術後創部に局所投与する事で、その創傷治癒促進効果および感染性制御効果により、大切断や皮弁壊死の回避を試みる。

Non-healing wounds with diabetic mellitus and/or peripheral artery disease (PAD) show high rates of complications, and a major risk factor for lower extremity amputation. In these cases, local tissue hypoxia, caused by disrupted or compromised vasculature, is a key factor that limits wound healing and topical infection control. Reconstructive surgical techniques to close these wounds frequently suffer serious complications, including flap necrosis and infection.
Oxygen plays an essential role in the metabolism, the matrix synthesis, infection control and cell migration and proliferation. Topical oxygen application might be effective to promote wound healing and suppress the bacterial growth.This study aims to examine the clinical efficacy of topical oxygen therapy in patients with non-healing ulcers and/or postoperative wounds
