選択基準/Key inclusion criteria

1. Chronic kidney disease（CKD）のステージが1，2，4，5のいずれかに相当する患者
2. CKDステージ3の場合は蛋白尿を有し，かつ糖尿病または高血圧を有する患者

1. Patients with CKD stage 1,2,4 or 5.
2. Patients with CKD stage 3 who have proteinuria, also with diabetes mellitus or hypertension

除外基準/Key exclusion criteria

1. 透析患者
2. 本研究に参加する腎臓専門医のみでCKDの治療を受けており，かかりつけ医には通院していない患者
3. 本研究に参加するかかりつけ医・腎臓専門医以外でCKDの治療を受けている患者

1. Dialysis patients
2. Patients who undergoes treatment of CKD only at nephrologists, not at general practical physicians
3. Patients who undergoes treatment of CKD at physicians who do not register this study
